| 目的：探讨利用药品监管码在医院门诊药房建立中成药溯源系统的可行性，以降低相似中成药调剂差错率。方法：将药品监管码嵌入医院信息系统（HIS），改造门诊药房处方调剂流程，在配药确认处增加监管码匹配、审核步骤；通过记录监管码，结合HIS中病人的信息，实现药品溯源的目的。结果：建立了中成药从医院门诊到病人之间的溯源系统。实施6个月后，中成药相似药品调剂差错下降，差错率从模式运行前的0.0523‰降为0.0181‰，下降了65.4%。病人候药时间没有明显增加。结论：利用药品监管码建立中成药末端溯源系统，可使药品溯源链得到完善，减少了门诊药房相似药品调剂差错事件。 |

| Objective: To establish traceability system of traditional Chinese patent medicines using bar code technology in outpatient pharmacy of hospital so as to reduce dispensing errors of similar traditional Chinese patent medicines. Methods: The dispensing process of prescriptions in outpatient pharmacy was modified by embedding the drug regulatory code into the hospital information system (HIS). A matching and checking process of drug regulatory code was added onto the dispensing confrmation. The purpose of drug traceability was achieved by recording the drug regulatory code, combined with the patient information in HIS. Resuults: The traceability system of traditional Chinese patent medicines from outpatient pharmacy to patients was established. The dispensing errors of similar traditional Chinese patent medicine were decreased 6 months after the implementation of the traceability system. The error rate reduced from 0.0523‰ before the implementation to 0.0181‰ after the implementation, decreasing by 65.4%. But there was no signifcant increase in patients' waiting time. Conclusion: The established drug traceability system using drug regulatory code could improve drug traceability chain and reduce the dispensing errors of similar traditional Chinese patent medicines in outpatient pharmacy of hospital. |
