目的:分析新型冠状病毒感染患者抗菌药物应用情况,为临床合理使用抗菌药物提供参考。方法:采用回顾性调查方法,统计北京市垂杨柳医院2022年12月1日-2023年1月15日确诊为新型冠状病毒感染患者的临床资料,抗菌药物使用品种、疗程、联合用药等,分析抗菌药物使用合理性,并对数据进行统计分析。结果:1011例新型冠状病毒感染患者中363例使用抗菌药物,抗菌药物使用率为35.91%, 抗菌药物联合使用率为11.85%。使用抗菌药物患者年龄中位数为76岁(64.5~84),79.61%患者合并 1种以上基础疾病。抗菌药物使用DDDs合计3571,其中使用DDDs最高的为β内酰胺类/酶抑制剂1153 (32.29%),使用率最高的为头孢菌素类183例(50.41%)。363例使用抗菌药物的患者中有27例使用不合理(7.44%),其中13例(3.58%)为无适应证用药,6例(1.65%)为联合用药不适宜,4例(1.10%) 为用法用量不适宜,2例(0.55%)为溶媒浓度不适宜。结论:新型冠状病毒感染患者抗菌药物使用过程中存在一定的不合理使用情况,临床应严格把握抗菌药物使用指征,加强病原学监测,避免抗菌药物过度治疗。 |

Objective: To analyze the application of antibiotics in patients with coronavirus disease 2019 (COVID-19), and to provide reference for rational use of antibiotics in clinic. Methods: A retrospective investigation method was used to analyze the clinical data of patients diagnosed with COVID-19 in Beijing Chuiyangliu Hospital from December 1, 2022 to January 15, 2023, as well as the varieties, courses, and combination of antibiotics used, etc. And the rationality of the use of antibiotics was analyzed, the data were statistically analyzed.Results: Among 1011 cases of COVID-19, 363 cases used antibiotics, the utilization rate of antibiotics was 35.91%, and the combined use rate of antibacterial drugs was 11.85%. The median age of patients using antibiotics was 76 years old (64.5~84), and 79.61% of patients had more than one underlying disease. The DDDs of antibacterial drug use totaled 3571, of which 1153 (32.29%) were β-lactam/enzyme inhibitors, 183 (50.41%) were cephalosporins. Among the 363 patients using antibiotics, 27 cases were irrational (7.44%), among which 13 cases (3.58%) were insuffi cient drug indications, 6 cases (1.65%) were inappropriate drug combination, 4 cases (1.10%) were inappropriate usage and dosage, and 2 cases (0.55%) were inappropriate solvent concentration. Conclusions: There are some irrational uses of antibiotics in patients with COVID-19. In clinical practice, the indications of antibiotic use should be strictly grasped, etiological monitoring should be strengthened, and excessive treatment of antibiotics should be avoided. |
