目的:运用失效模式与效应分析法探究青岛大学附属医院血透室用药安全管理现状,以提升医院血透服务质量,提升血透患者就医体验。方法:对血透患者药品领用流程中各环节进行失效模式与效应分析(Failure Mode and Effect Analysis,FMEA)。选出全部中等及以上失效模式整理分析其失效原因, 提出并实施整改措施,再对整改后的各失效模式进行FMEA。结果:对选出的16项中等及以上风险等级的失效模式进行整改后,风险优先指数由828降至222,降幅73.2%。结论:运用失效模式与效应分析干预医院血透患者用药安全,可提高医院血透专科工作质量和服务水平,降低医疗风险。 |

Objective: To improve the drug safety management of the hemodialysis units of the Affiliated Hospital of Qingdao University, the quality of hospital hemodialysis service, and the medical experience of patients by using failure mode and effect analysis (FMEA). Methods: FMEA were used in each part of the drug receiving process for hemodialysis patients. All medium and above failure modes were selected and analyzed for causes, improvement measures were proposed and implemented, and then FMEA was conducted for each improved failure mode. Results: After improving the selected 16 failure modes with medium and above risk levels, the total RPN value decreased from 828 to 222, a reduction of 73.2%. Conclusion: Using failure mode and effect analysis to intervene in the drug safety of hospital hemodialysis patients could improve the work quality and service level of hemodialysis department and reduce medical risks. |
