| 目的：降低中药配方颗粒智能调配差错风险。方法：应用医疗失效模式与效应分析方法（HFMEA）对中药配方颗粒智能调配过程中的失效模式（FM）进行识别分析，对FM的严重程度、发生概率、可检测性进行量化评估，计算风险优先指数（RPN），将实施HFMEA前后的RPN进行配对t检验。结果：实施HFMEA措施后，影响中药配方颗粒智能调配的失效模式的RPN值由86分降到23分，下降了73%，（P=0.012），差异具有统计学意义（P<0.05）。结论： HFMEA应用于中药配方颗粒智能调配差错的风险管理，可有效降低差错，保证用药安全。 |

| Objective: To reduce the risk of error in the intelligent dispensing of traditional Chinese medicine granules. Methods: The method of Healthcare Failure Mode and Effect Analysis (HFMEA) was used to analysis and identify the failure mode (FM) in the process of intelligent dispensing of traditional Chinese medicine granules.The severity, occurrence and detectability of FM were quantitatively assessed, and the risk priority number (RPN) was calculated. RPNs before and after HFMEA were analyzed by using paired t-test. Results: After the implementation of HFMEA, the RPN value of the failure mode that affected the intelligent dispensing of traditional Chinese medicine granules reduced from 86 points to 23 points, which decreasing by 73%, P=0.012, and the difference was statistically significant (P<0.05). Conclusions: HFMEA applied to the risk management of intelligent dispensing of traditional Chinese medicine granules can effectively reduce errors and ensure medication safety. |
