| 目的：了解华南农业大学医院门诊处方不合理情况，促进临床合理用药，保障患者用药安全。方法：随机抽取我院2016年门诊处方，每月各100张，共计1200张，对不合理处方进行统计分析。结果：在抽查的处方中，不合理处方共94张，处方合理率为92.17%。不合理处方包括不规范处方20张（21.28%），不适宜处方58张（61.70%），超常处方16张（17.02%）。不合理处方主要集中在临床诊断与用药不符、联合用药不适宜、抗菌药物应用不合理等方面。结论：门诊处方总体用药情况较合理，但仍存在一些问题，应进一步加强处方管理，促进安全、有效、经济用药。 |

| Objective:To investigate the irrational prescriptions in outpatient departments of the Hospital of South China Agricultural University (referred to "our hospital"), so as to improve clinical rational drug use and to ensure the safety of medication for patients. Methods:A total of 1200 outpatient prescriptions were randomly selected from our hospital in 2016 (100 prescriptions per month). The irrational prescriptions were analyzed. Results:Among the 1200 prescriptions, 94 were irrational prescriptions, the rational rate was 92.17%. Of the irrational prescriptions 20 (21.28%) were irregular prescriptions, 58 (61.70%) were unsuitable prescriptions and 16 (17.02%) were off-label prescriptions. The irrational prescriptions were found mainly in the following aspects:discrepancy between clinical diagnosis and medication, improper drug combination and irrational use of antibiotics. Conclusion:The overall outpatient prescriptions in our hospital are generally rational, but some problems still exist, which need to be solved. The prescription management should be further strengthened to promote safe, effective and economical medication. |
