| 目的：分析临床药师的用药咨询记录，为指导患者安全、合理用药提供参考。方法：回顾性整理2016年临床药师书写完整的药物咨询记录，从咨询人员构成、咨询问题分类、咨询药物种类、给药途径4个方面进行统计分析。结果：药物咨询人群以患者及其家属居多；咨询问题类别多与特殊人群用药、药物选择、药物不良反应等有关；药物种类主要是以临床常见的慢性病用药为主；咨询问题给药途径以口服为主，其次是吸入制剂与腔道用药。结论：临床药师需要根据咨询中遇到的常见问题不断提高自身知识储备，掌握医学、药学、检验、营养学知识，提高沟通技巧等方面的能力，在实践中不断完善。 |

| Objective: To analyze medication consultation records of clinical pharmacists so as to provide references for the safe and rational drug use. Methods: The medication consultation records written by clinical pharmacists in 2016 were retrospectively analyzed. The patient composition, classification of consultation questions, types of drugs and routes of administration were statistically analyzed. Results: A majority of the medication consultation population was patients and their relatives. The main classifications of consultation questions were medication in special populations, selection of drugs, adverse drug reactions and so on. The types of drugs mainly included drugs for treating chronic diseases. The route of administration was mainly oral, followed by inhalation and ligation. Conclusion: Clinical pharmacists should improve their knowledge relevant with the common problems encountered during the medication consultation, master skills of medicine, pharmacy, testing, nutriology, communication, etc. and continuously improve the above-mentioned knowledge and skills in practice. |
