| 目的：对中药注射剂不合理医嘱进行统计分析，探讨中药注射剂临床用药的合理使用。方法：对湖南省常德市第一人民医院静脉药物配置中心（简称静配中心）于2016年6月至8月审核处方过程中发现及纠正的中药注射剂不合理医嘱进行分析总结。结果：共审核2207份中药注射剂医嘱，其中不合理医嘱46例，不合理医嘱主要为溶媒选择与用量、药物剂量、联合用药、配伍用药及用法不合理。结论：静配中心的建立对中药注射剂的合理使用起到了应有作用，而不合理医嘱的数据也反映出还存在一定问题，需要临床医师、静配中心药师以及护士的审核等环节进一步改进、完善，从而确保患者用药安全。 |

| Objective: To analyze the irrational prescriptions of traditional Chinese medicine injections in our hospital and discuss the clinically rational use of them. Methods: The irrational prescriptions of traditional Chinese medicine injections from June 2016 to August 2016 were analyzed and summarized in intravenous medicine preparation center of the First People's Hospital of Changde in Hunan Province. Results: There were 46 irrational prescriptions out of 2207 reviewed prescriptions of traditional Chinese medicine injections. The major problems of the irrational prescriptions were as follows: irrational selection and dosage of vehicles, irrational medicine dosage, irrational combination of medicines, irrational compatibility of medicines, irrational usage of medicines as well as input error. Conclusion: The establishment of intravenous medicine preparation center played a proper role in the rational use of traditional Chinese medicine injections. However, the irrational prescription data reflected the existence of some problems, which need to be further solved by clinicians, pharmacists of intravenous medicine preparation center, nurses and so on, so as to ensure the medication safety of patients. |
