| 目的：调查西藏自治区人民医院糖皮质激素药物的使用情况，并对其使用的合理性进行评价分析，为临床合理用药提供参考。方法：调取与统计医院信息系统(HIS)2016-2019年糖皮质激素药物的使用数量及金额数据，并采用分层抽样法抽取其中包含糖皮质激素药物的门诊处方1657张及住院医嘱 1412份，以药品说明书及临床用药指南等作为点评依据，对其用药合理性进行统计分析。结果：2016- 2019年我院使用的糖皮质激素药物共27种，使用数量及金额排序前五名的品种连续四年未发生变化。门诊不合理处方382张，不合理率为23.05%；住院不合理医嘱289份，不合理率为20.47%。不合理原因与存在主要问题包括用法用量不适宜、重复用药、存在禁忌症、超说明书用药等。结论：我院糖皮质激素药物使用尚存在不合理性，应加强对糖皮质激素药物临床应用的管理，以促进临床合理用药。 |

| Objective: To investigate the use of glucocorticoid drugs in the People's Hospital of Tibet Autonomous Region, evaluate and analyze the rationality of their use, so as to provide references for clinical rational use of these drugs. Methods: The HIS system of the hospital was used to calculate the quantity and amount data of glucocorticoid drugs used in the hospital from 2016 to 2019. 1657 outpatient prescriptions and 1412 inpatient orders of glucocorticoid drugs in this hospital were selected by stratified sampling method. The drug instructions and clinical medication guidelines were used as the review basis and the rationality of glucocorticoid medication was statistically analyzed. Results: 27 kinds of glucocorticoid drugs were used in this hospital from 2016 to 2019. The top five varieties in terms of quantity and amount of use did not change for four consecutive years. There were 382 unreasonable prescriptions in outpatient department, and the irrational rate was 23.05%. There were 289 unreasonable medical orders in hospital, and the unreasonable rate was 20.47%. The unreasonability and the main problems include inappropriate usage and dosage, repeated drug use, contraindications and off-label use of drugs. Conclusion: The use of glucocorticoid drugs in our hospital is stillirrational, and the management of clinical application of glucocorticoid drugs should be strengthened in order to promote rational clinical use of drugs. |
