| 目的：探讨社区医院药师和医师联合开设门诊进行药学服务工作模式的经验和不足，并分析社区医院药学服务对象的特征，为社区医院药学服务转型提供思路和实践经验。方法：结合具体案例回顾性探讨分析医师药师联合门诊的构建和实践，以及2019.6-2020.8期间就诊患者的数据。结果：医师药师联合门诊的前期总体设计和准备工作是开展药学服务的基础，药学服务的保障是建立药学服务标准流程。 医师药师联合门诊就诊患者以60岁以上老年患者为主，多数患者存在联合用药，原发病以慢性病为主， 属于药物不良反应(ADR)的高危人群。该门诊的开设不仅促进了合理用药，并且推动了社区医院ADR 监测工作。结论：社区医院开展医师药师联合门诊是实现药学服务转型、拓展药学项目的有效手段，能够给患者提供专业的药学服务，促进提升药师能力，完善诊疗工作和减轻医生压力，实现患者、药师和医生三方共赢。 |

| Objective: To explore the experiences and deficiencies of pharmacy service pattern with a joint outpatient service combining doctors and pharmacists, and to analyze the characteristics of community hospital pharmacy service objects, so as to provide ideas and practical experience for the transformation of pharmaceutical care in community hospitals. Methods: Retrospectively explore and analyze the establishment and practice of joint outpatient service by doctors and pharmacists with specific cases, as well as the data of patients who visited the clinic from June, 2019 to August, 2020. Results: The preliminary overall design and preparation of the joint outpatient service is the basis of pharmacy services, and the guarantee of pharmacy services is to establish a pharmaceutical service standard process. Most of the patients treated by physician-pharmacist joint outpatient department are elderly patients over 60 years old. Most of the patients have drug combination, and the primarydisease is mainly chronic diseases, which belongs to the high-risk population of adverse drug reactions (ADR). The establishment of the outpatient service promotes not only rational drug use, but also community hospital ADR project. Conclusion: The joint outpatient service of physicians and pharmacists in community hospital is an effective method to realize the transformation of pharmaceutical care and expand pharmaceutical projects. It can provide patients with professional pharmacy services, promote the improvement of pharmacist’s ability, improve the diagnosis and treatment work and reduce pressure on doctors, which achieves a win-win situation among patients, pharmacists and doctors. |
